NDIS Participant Referral Form
Whilst we have re-opened our books to new NDIS participants, unfortunately we are not accepting referrals with a primary diagnosis of ASD or Psychosocial Disabilities. We feel these clients are best supported by agencies who have the specialty skills required for assisting people with these diagnoses. We feel it's essential that not only the participant feels we are a good fit for them but also that we feel we are the right OTs for them, given the long-standing nature of engaging with a NDIS Participant therefore prior to accepting referrals, we require the NDIS Participant Referral Form be completed.
As an approved provider under the National Disability Insurance Scheme (NDIS), Live Well OT can support you with determining what you need to consider within your pre-planning meeting as well as assist you with achieving your goals once your plan has been established.
